Student Progress Report (4/09)

Example for BEGINNER PLUS Student

  Student Name:

Josephina Chang

Date:

March 29, 2009

  Tutor Name:

Myra Jones

Learning Objectives:

  1. Make a doctor's appointment.
  2. Do the family's marketing.
  3. Speak to her son's teacher / school about his absences.

Description of Progress:

  1. Doctor's appointment: Josephina was able to call me at home and practice asking for an appointment with a gynecologist. We discussed her introducing herself and saying she had little English skills, and wanted to see the doctor for a pregnancy exam. We used a calendar to familiarize her with the months, days, and a clock picture for hours.
  2. Marketing: Josephina's lessons included a market scene of vegetables and fruits that we could identify by type, color, and quantity.
  3. Speak to Teacher / School: Used a Pictionary to describe parts of the body in English, to enable her to call the school to excuse her son's absence due to illness.

Summation: Descriptive Example:  Objectives Met /or Not Met, and why

  1. Doctor's appointment: Josephina had some difficulty in hearing my English when I pretended to be the receptionist and spoke quickly.  She is practicing to speak up and to quickly identify herself “my name is Josephina Chang and I have very little understanding of English”, and “may I please have an appointment with the doctor for a pregnancy exam”.  If I interrupt her with asking her to hold on, she does get anxious but we will continue to work on this.
  2. Marketing: Josephina is very good with colors and names of vegetables and fruits.  She also understands quantity and monies.
  3. Speak to Teacher / School:  Josephina is very good with body part names, less so with symptoms, but can call the school to report her son “sick”.
